WHAT TO EXPECT AT A VISIT.
When you come for your first appointment, we will perform a thorough examination and create a personalized treatment plan for you. We do our best to keep the paperwork pain-free and offer online intake forms prior to your appointment. If your insurance provider requires a co-pay, we will ask for this payment at each visit. We accept payments by cash, check, or credit card. Your subsequent visits will focus on various treatment techniques that are based on your diagnosis and individualized goals.
WHEN TO ARRIVE.
We ask that you please arrive 10 minutes prior to your appointment to sign-in, complete any necessary paperwork and/or change your clothes if desired. On average, the first appointment lasts about an hour.
WHAT TO BRING.
We ask that you bring your physician referral or prescription (if needed), insurance card, Valid ID and co-payment (as applicable). We recommend that you wear comfortable clothing with a loose fitting or stretchy material that could roll up as it may be helpful for us in order to fully examine the affected area.
